Return Merchandise Authorization Form

In order to expedite the RMA process, please follow the following guidelines, and be sure to fully complete the form below.

  1. A valid serial number must be given to qualify for return
  2. A 40% restocking fee will apply to products returned in working condition.
  3. For faster processing, please clearly mark the package as an RMA, by writing RMA on the box with a bold black marker.
  4. A RMA authorization doesn't guarantee that credit or replacement is approved.  Credits and replacements will be approved after each product is inspected and serial numbers matched.
  5. RMA are processed as soon as possible.  Please allow up to 10 days for complete processing.
  6. RMAs for DOA (Dead On Arrival) products must be submitted within 10 days of delivery to qualify for warranty replacement.
